Home Based Primary Care - Native American Program

Four days a week, the Home Based Primary Care Mobile Outreach team from McGuire VA Medical Center head out to various locations throughout central Virginia to provide medical care and services to senior Veterans and Native American Veterans living in rural areas. the surrounding areas.
The team uses a 40’ mobile clinic which is outfitted to perform most of the same functions as you would see at your local doctor’s office.
Since the establishment of the Mobile Team, the mobile clinic patient list continues to grow.

Services you can expect when you visit the mobile clinic:
Enrollment – The team brings an Enrollment Specialist to make sure each Veteran is aware of their benefits and to assist with the application process.
Social Work – Health professionals provide initial and continued assessment of patients and caregiver needs.
Nursing – Our Nurse Educators provide care and health education. They can assist Veterans with making healthier lifestyle choices to enhance their wellness.
Pharmacy – Help Veterans manage chronic conditions through education and medication adjustment.
